Chronic Disease Self-Management Program offered in Surprise
If you are struggling with a chronic disease, sign up today for the free Surprise Chronic Disease Self -Management Program.
This program, developed at Stanford University, will be held on Mondays, April 13 – May 18, from 1 p.m. - 2:30 p.m., at the Sierra Montana Recreation Center, 14861 N. Spring Lane.
Co-sponsored by the Area Agency on Aging and the Surprise Community and Recreation Services / Senior Services, the Chronic Disease Self-Management Program is a workshop for people age 18 and over that are coping with a chronic disease.
The workshop will teach day-to-day management skills and provide tips to maintain and/or increase a person’s activities. This course seeks to provide an interactive learning environment in which the participant can practice and master self-management techniques that positively affect quality of life factors.
A recommended companion book will be available in class for $21.00. Class size is limited and pre-registration will be required. To register please visit the Surprise Senior Center, 15832 N.
